Location: West End, North Carolina Facility: 40 - Regional Services Department: FIRSTHEALTH HOME CARE (TAYLORTOWN) Schedule: Full Time: 40 hrs/wk Shift: Day Shift Hours: 8am-5pm Job ID: 2024-16691

Overview

Accurately enters all appropriate patient information into EMR

Schedules all PT and SN admissions and evaluations

Coordinates scheduling of all home health visits

Monitors compliance with Therapy Functional Assessments and supervisory visits

Monitors staff productivity and communicates with Clinical Managers

Completes any required audits

Triages Intake phone calls and referrals

Assists the Intake Department as needed

Communicates request for authorizations with appropriate personnel

Qualifications

High School graduate required. Associates Degree preferred. 2-3 years of Home Health experience preferred. Excellent computer skills required.
