Home Care RN Job Summary




  • The RN Case Manager is responsible for the clinical management of an assigned patient population.

  • The RN Case Manager develops a patient centered, comprehensive plan of care utilizing a multidisciplinary team approach. 

  • The RN Case Manager is accountable for achieving positive patient outcomes

  • Supervises Home Health Aides and LPN’s according to policy

  • Reports daily to the Clinical Manager all pertinent patient information

  • Reports/records information accurately

  • Uses appropriate safety and infection control measures





Home Care RN Qualifications




  • Graduate of an accredited school of nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact.

  • Minimum of one (1) year of acute care or home care experience required. Evidence of current CPR certification required.

  • Valid North Carolina Driver’s License and proof of auto liability insurance required.
