Position Summary
FirstHealth of the Carolinas is a nationally recognized health care system located in central North Carolina. Comprised of four hospitals (Moore, Hoke, Richmond, and Montgomery) with more than 600 beds, the system also offers leading-edge heart care in the Reid Heart Center, the area’s only dedicated heart and vascular center. Hospice House Social Worker Job Summary
- Responsible for provision of social work services to include crisis intervention, advance care planning conversations, resource referral and emotional support and/or counseling to patients and families as needed.
- Ability to work in coordination with other disciplines at part of team to address needs related to serious illnesses.
Hospice House Social Worker Job Requirements
- Masters Degree in Social Work with NC LCSW or LCSW Provisional license required.
- At least 1 year of social work experience in any field.
- Knowledge of end-of-life issues preferred.
- Reliable transportation and valid NC Drivers license required.
